Medical Director: Oversees the plan of care and works collaboratively with the patient's personal physician and the hospice interdisciplinary team.

Hospice Aide: A Certified Home Health Aide (CNA/CHHA) who assists with daily living activities, such as personal grooming, bathing, or just taking the time to sit and talk.

Social Worker: Provides emotional support and counseling, as well as assistance in dealing with end-of-life concerns and accessing community resources that can provide additional help.

Case Manager: A registered nurse that is specially trained to provide comfort care and pain management. Coordinates responses to patient needs with all members of the hospice team, physicians, nursing home and assisted living staff and caregivers.

Licensed Vocational Nurse: Collaborates with our registered nurse to closely monitor and manage all symptoms to provide for our patients’ comfort and peace of mind.

Spiritual Care Coordinator: Ensures that spiritual needs are addressed and, when needed, acts as a liaison between patients and their faith community.

Volunteer: Specially trained, caring individuals dedicated to providing companionship and support (see volunteer page for additional information).
