A Hospice Team That Cares

At LightBridge, our knowledgeable
team of specially trained Hospice
professionals works to continually maintain a personalized program of care that meets each patient’s unique needs. We coordinate
our services to complement the
care already being provided by
family, friends, and the medical professionals our patients
know and trust.

Medical Director

Oversees the plan of care and
works collaboratively with a
patient’s personal physician.

Case Manager

A registered nurse that is
specially trained to provide
comfort care and pain
management. Coordinates
responses to patient needs
with all members of the hospice
team, physicians, nursing
home and assisted living
staff, and care givers.

Home Health Aide

Assists with daily living activities,
such as personal grooming,
bathing, or just taking the time
to sit and talk.

Social Worker

Provides emotional support
as well as assistance in dealing
with end of life concerns and
accessing community resources
that can provide additional help.

Spiritual Care Coordinator

Ensures that spiritual needs are
addressed and, when needed,
acts as a liaison between
patients and their faith community.

Volunteer

Specially trained, caring
individuals dedicated to providing
companionship and support.
